I give my permission for Girl Scouts of Michigan Shore to Shore to conduct a criminal background check and personal history. The record received from reporting agencies may include arrest and conviction data, plea bargains and deferred adjudication. It may also include information regarding my driving history.
I understand that this information will be used, in part, to determine my eligibility for a volunteer position with this organization. Girl Scouts of Michigan Shore to Shore has the right, in its sole decision, to reject this application and deny admission as a volunteer based on the results of the investigation.
I expressly waive any claim against the Girl Scouts of Michigan Shore to Shore and its officers, directors, employees, and agents which may arise from the investigation, use of the information discovered, and the decision to accept or reject this application.
I also understand that as long as I remain a volunteer, the criminal background check may be repeated at any time.
I hereby release and agree to indemnify and hold Girl Scouts of Michigan Shore to Shore, and each of their employees and agents, harmless from and against any and all liability, expense and claims for damage of any nature resulting from the investigation of my background check.
I certify that all information provided on this application is true and complete. I understand that falsification or significant omissions of any information may be considered justification for non-acceptance, or dismissal at a later date.
